Ohio was a state of the pre-War United States of America.
Background
The Silver Bridge was built in 1928 as the first eye-bar suspension bridge in the U.S. It served the communities of Point Pleasant, West Virginia and Gallipolis, Ohio until December 15, 1967, when it collapsed.[1] Orvilletown, Ohio was the home of the L.S. Smuckman Company, which manufactured industrial size shortening.[2]
A "Storm of 2048" affected southeastern Ohio, causing landslides, one of which caused irreparable damage to the historical site of the Athens Lunatic Asylum near Athens.[3] Additionally, parts of eastern Ohio suffered damage from wildfires sometime in the 2050s, leading to an abnormal increase in populations of fire chaser beetles in the affected areas. Researchers concluded that the infrared radiation emitted by the fires was influencing their heat detection mechanisms; the assumption was that, without adequate pest control measures, they would eventually pose a threat to local agriculture.[4] According to the Ghoul, the arid desert biome found in the Burning Springs region of the state by 2105 does not resemble the condition it was in pre-War.[5]
The state is shown to still be inhabited after the bombs dropped. Initiate Norland claims it as his home before he was recruited to the Brotherhood First Expeditionary Force when they passed through heading to Appalachia.[6] A group of raiders that traded with the Crater Raiders were said to have come from the west and passed through Ohio on their way to Appalachia.[7]
Burning Springs
In the aftermath of the Great War, a sizable portion of the state's southeastern area was dramatically altered by a phenomenon known as "the Rust" that wiped out significant amounts of plant life. While rumor suggests it was not as dramatic in other areas and there has been regrowth by 2105, the region on the border of Appalachia, now known as Burning Springs, was never able to recover and remained a desert for decades after.[8] In truth, chemical spills from Abraxodyne Chemical, a company that had a significant presence in the region before the bombs due to its research into Project Meadow Breeze, caused the Rust.[9] This affected many of the Springs' residents with a serious illness, with prominent symptoms including heavy coughing and thirst.[10]
Immediately after the War, members of the Ohio National Guard initially attempted to keep order in Athens while the Rust took its toll on the area, with sick individuals dying and being disposed of in burn pits, while others became feral ghouls.[11][12] Seismic activity in the aftermath of the bombs caused further damage to the city.[13] As much of the civilian population dwindled, either by death or fleeing the area,[14] the National Guardsmen became affected by the Rust as well, and were forced to abandon the city as the nightmarish situation in Athens became "untenable."[15]
The political landscape of post-War Ohio changed significantly around the time an Enclave transport vertibird carrying a sedated super mutant crashed south of Athens, allowing the mutant to escape his imprisonment.[16] Possessing unusual intelligence while still having a hefty bloodlust, this super mutant sought battle after battle and eventually attained a reputation and self-proclaimed title as the Rust King, with a goal to conquer the area. He formed the Rust King's Army, with many of his followers derived from gangs of violent raiders. They established the Rust Kingdom in an old Abraxodyne facility.[17] From there, the Rust King's forces went on to conquer Fort Steuben and construct Checkpoint Canyon to assert the boundaries of his territory, establishing dominance over the region.[18][19]

Notes
- The raider punk alleges that the Vault 76 Vault Dwellers may be taken by aliens and dropped in Ohio or worse if a sacrifice is not provided to the aliens.[20]
- In one of the sacred tomes, specifically Observations, vol. XXVII inside the Point Pleasant church, it mentions that the writer was in "Woods - Ohio?"
- The city of Columbus is mentioned several times throughout Burning Springs, including having a stadium known as The Horseshoe that was reduced to a crater during the Great War.[21]
Behind the scenes
Overall, the Burning Springs region is a segment of the real-world area known as Appalachian Ohio. According to producer Bill LaCoste, the real-world range of Burning Springs could be compared to "south of Point Pleasant/Gallipolis, west to about Dexter [City] maybe, and up north to Athens. So just a really small SE portion of it."[Non-game 1]

During the Storm of 2048 the building was hit by a major landslide and was damaged beyond repair. The Ghoul: "Ohio? Didn't think much of it before the bombs dropped, and it hasn't improved since. Never expected a desert out this way, though. Somebody really fucked it up."
(The Ghoul's dialogue) - ↑ Vault 76 dweller: "Pleased to make your acquaintance, Initiate. Do you mind introducing yourself?"
Norland: "Sure, I can introduce myself. I come from Ohio. My parents worked in the government before the War. When I learned about the Brotherhood and what they're doing to preserve the world, I left home to do my part. Make up for my parents' contribution to causing this disaster, however small it was. It's a big responsibility, so I don't have time for much else. I need to be vigilant about my training if I'm going to become a Knight."
(Norland's dialogue) - ↑ Vault 76 dweller: "The other Raiders you got the weapons from... Where did they come from?"
Sheena: "West. They said they passed through Ohio before coming here. I don't know how much further out they went."
(Sheena's dialogue) - ↑ Vault 76 dweller: "What do you know about the Rust?"
Pop: "What does anyone know? It's death itself. It came after the Bombs, and took all the plants with it. I heard things started growing back in other places, but never here. Folks treat it like some kind of force of nature, but if you ask me? It ain't nature, it's us."
